Group Membership

Group ID is required for all groups.  Information needed to receive a Group ID:

1.       Name of each group member

2.       Contact information for each group member (mailing address, email address and telephone number)

3.       Individual who is to be lead group member (this individual will be the name on the ballot for voting in September)

(Note: New for 2012 we are adding a small adminstrative fee to cover the additional cost associated with multiple members,)

       -   Duet Group membership fee is $525/person if mailing a check, $538 if paying online. 
       -   Trio Group Membership is $364/person if mailing a check, $376 if paying online. 
      -    Quartet Group membership is $290/person if mailing a check, $297 if paying online.
