¨ Designate the focus area you are applying to and complete organization data in Section1 of the Common Grant Application ¨ Describe the program/project in Section 2-6 on five single-sided pages or less, using 12 font letters. ¨ Agency Audit (provide one) for 2009, 2008 and 2007 ¨ Audit and opinion letter OR ¨ Non-audit review (review, compilation, or company prepared) OR ¨ Form 990 ¨ Financial Statements to be included for each fiscal year ¨ Fiscal years to be included ¨ Current Year-to-date ¨ 2009 ¨ 2008 ¨ 2007 ¨ Type of Financial Statements to be included ¨ Income Statement ¨ Balance Sheet ¨ Cash Flow ¨ Agency Budget ¨ Current fiscal/calendar year ¨ Year-to-date status ¨ Project Budget ¨ Complete project budget ¨ Next two (2) years pro forma project budget ¨ Provide a copy of 501 (c) (3) designation by IRS ¨ Agency Governance (roster of officers and members of the Board of Directors) ¨ Board positions/committees and terms of services ¨ External employment firm and position ¨ Include signed Terms of Agreement document
